The Bills have agreed to a 4-year, $44 million extension with LB Matt Milano, according to NFL Network's Ian Rapoport. The deal includes $24 million guaranteed. It obviously locks Milano in as a full-timer for at least the next couple of seasons. Milano struggled through hamstring and pectoral injuries last season that limited him to just 3 games of at least 50% playing time. It's particularly telling that Buffalo paid up in spite of that. Milano carries top-20 IDP scoring upside at his position.
The Bills have made CB Tre'Davious White, DE Jerry Hughes and DE Mario Addison healthy scratches vs. the Dolphins today. That points to the team planning to rest starters in general over playing for the win. Keep that in mind if you've been planning to start QB Josh Allen or WR Stefon Diggs in a season-long lineup.
The Bills have activated LB Matt Milano from IR (pectoral) and plan to play him against the 49ers tonight. Coaches haven't revealed details on his role, though. HC Sean McDermott pointed out early in the week that A.J. Klein has been the one playing while Milano has missed weeks. That sounds like a coach hesitant to return even a healthy Milano to full playing time. It's also worth noting that MLB Tremaine Edmunds has scored poorest among Bills LBs across the board in Pro Football Focus grading. Klein, meanwhile, holds a higher coverage grade than even Milano right now and rates among the team's best pass-rushers. If you've been planning to use Klein in your Week 13 IDP lineup, we'd stick with him. We'll see about the role tonight for Milano, who also played limited snaps the 2 games before landing on IR while trying to manage the injury.
Bills S Micah Hyde (ankle) is active for today's game against the Cardinals. He's merely a low-level IDP option but available if you were waiting for him.
The Bills have placed CB Josh Norman on the COVID list after he tested positive. Buffalo has also placed TE Tyler Kroft, CB Levi Wallace and S Dean Marlowe on the list as close contacts. They won't travel for Sunday's game against the Cardinals. That removes a starting corner (Wallace) and the potential replacement for S Micah Hyde if Hyde turns up inactive. He's questionable after sustaining an ankle injury Thursday that kept him out of Friday's practice. So Buffalo could be facing Arizona down 2 of its top 3 outside CBs and starting a 3rd-stringer at safety.

Milano played a limited role each of the past 2 weeks because of a pectoral injury that cost him the previous 2 games. This week's change at least keeps us from having to guess at how to treat Milano in IDP lineups. A.J. Klein has been the 2nd Bills LB in playing time when Milano has missed time (behind MLB Tremaine Edmunds).

Hyde looks unlikely to play after missing practice all week. Hughes and Jefferson each put in a full practice Friday and looks likely to be available. Milano played through a questionable tag last week and continued to practice on a limited basis this week. He played just a fraction of his normal role in Week 7, though. We have to assume he won't be back to full playing time this week.
